  • A photo of your ID (could be a passport, driving license or workplace ID card)
  • A list of your medical conditions and the medications you take
  • A short video stating only your first name and confirm the service requested (e.g. My name Anna and I am requesting a travelling with medication letter)**
  • (Optional) If you want us to contact your regular NHS GP about your consultation, have ready the details of your GP practice.
