What details do you need to provide?

For us to review your case and issue a doctor letter for exemption from an activity on medical grounds, we’ll need the following information.

A photo of your ID (could be a passport, driving license or workplace ID card)
Upload your health summary, medical history or medical documentation that you have received from a medical professional (this could be your GP health record). For guidance on how to obtain it click here

The activity, procedure or requirement that you request to be exempt from on medical grounds e.g. shaving exemption, travel exemption

A short video stating only your first name and confirm the service requested (e.g. "My name Eric and I am requesting a medical exemption letter")**

(Optional) If you want us to contact your regular NHS GP about your consultation, have ready the details of your GP practice.

Our medical exemption letters are valid for 12 months in cases of short term medical conditions, for long term medical conditions they are valid for 24 months.

Can I use this certificate for multiple exemptions?

The certificate will specify what you are exempt from, and it can cover multiple exemptions if they are related to the same medical condition.

Can I get a refund?

We only provide full refunds if we are unable to provide a medical letter.
Once we have reviewed your case and have issued a medical letter, we are unable to process a refund.
