What details do you need to provide?

For us to review your case and issue you with a medical letter verifying your injury,  we’ll need the following information.

  • A photo of your ID (could be a passport, driving license or workplace ID card)
  • Details regarding the injury or accident you sustained (when it happened, what areas of your body are affected)
  • A short video stating only your first name and confirm the service requested (e.g. My name Joe and I am requesting an injury or accident confirmation letter)**
  • (Optional) If you want us to contact your regular NHS GP about your consultation, have ready the details of your GP practice.
