What details do you need to provide?

For us to assess your case and write you a gym or health club cancellation letter,  we’ll need the following information.

  • A photo of your ID (could be a passport, driving license or workplace ID card)
  • Details about the sickness, injury or medical condition that has impacted your membership
  • The gym or health club you want us to write to
  • Whether you wish to cancel or suspend your membership
  • A short video stating only your first name and confirm the service requested (e.g. My name Oliver and I am requesting a gym cancellation letter )**
  • (Optional) If you want us to contact your regular NHS GP about your consultation, have ready the details of your GP practice.
