What details do you need to provide?

For us to produce a medical letter for your blue badge application,  we’ll need the following information.

  • A photo of your ID (could be a passport, driving license or workplace ID card)
  • Your medical disability or health condition
  • Your digital Summary Care Record (GP Health Record which can be accessed online via the NHS website) or a doctor’s letter confirming your medical disability
  • A short video stating only your first name and confirm the service requested (e.g. My name Anna and I am requesting a blue badge support letter.)**
  • (Optional) If you want us to inform your regular NHS GP about your consultation, have ready the details of your GP practice.
