To assess your condition and issue a medical certificate, we’ll need the following information.

Write down your symptoms and upload a brief video stating only your first name and the service you're requesting.
Our doctors will review the evidence you provide.
You will receive a verifiable digital letter signed by a GMC registered medical doctor if you order before 9 pm GMT - 7 days a week.
After you fill in our questionnaire to tell us about your symptoms your request will be reviewed by one of our doctors. If our doctors have any questions about your condition or symptoms, they may call you directly. Once your workplace medical sickness certificate is validated, it will be sent to you via email.
In some cases, our doctors may decide that you need to see your GP in person or visit A&E for urgent care. In these situations, we won’t be able to issue a certificate, but you’ll receive a full refund, no questions asked.
Our doctors will review your evidence and provide you with your certificate within the same day if you make your order before 9pm. Weekends included.
We provide medical certificates that are valid for 7 days from the date of issue. If needed, you can request your certificate to be backdated
Our fit notes are valid in most situations. Doctument provides private medical certificates issued by licensed UK doctors. However, these certificates differ from NHS Med3 Fit Notes, which are only available through NHS healthcare services.
We only provide full refunds if we are unable to provide a medical certificate.
Once we have reviewed your case and have issued a medical certificate, we are unable to process a refund.
