What details do you need to provide?

For us to review your case,  we’ll need the following information.

  • A photo of your ID (could be a passport, driving license or workplace ID card)
  • Details regarding when your illness or condition first started and if you have now recovered
  • A short video stating only your first name and confirm the service requested (e.g. My name Anna and I am requesting an event cancellation medical letter **
  • (Optional) If you want us to contact your regular NHS GP about your consultation, have ready the details of your GP practice.
