What details do you need to provide?

For us to review your case and issue a chickenpox recovery letter,  we’ll need the following information.

  • A photo of your ID (could be a passport, driving license or workplace ID card)
  • Confirmation on when the chickenpox first began, and that your spots have now scabbed over
  • Details about your airline and flight
  • A short video stating only your first name and confirm the service requested (e.g. My name Anna and I am requesting a chickenpox recovery letter)**
  • (Optional) If you want us to contact your regular NHS GP about your consultation, have ready the details of your GP practice.
